Failed an emissions / smog test — what “not ready” and a MIL really mean

Inspection stations are not diagnosing your car. They are checking two things: is the malfunction indicator lamp commanded on, and have the OBD2 readiness monitors completed? Fixing a part without watching those two flags is how people fail twice.

Fail type A: the check engine light is on

A commanded-on MIL is an automatic fail in most OBD inspection programs. Read the code. Do not clear it the night before a retest. Clearing resets readiness monitors to incomplete, which is fail type B.

High-volume inspection codes:

• P0420 / P0430 catalyst efficiency
• P0442 / P0455 EVAP leak (gas cap to tank)
• P0171 lean
• P0401 EGR insufficient flow
• O2 heater codes such as P0135

Fail type B: monitors not ready

After a battery disconnect or a Mode 04 clear, catalyst, EVAP, O2, and EGR monitors often show incomplete. A short drive to the shop is not a drive cycle. Continuous monitors (misfire, fuel) usually come back quickly; EVAP can take days of mixed driving.

What to do in order

1. Scan and save codes + freeze frame. Search the code on this site.
2. Repair the cause — not “clear and hope.”
3. Clear only after the repair, then complete a proper drive cycle.
4. Confirm the MIL is off and the required monitors are complete.
5. Then go back to the station.

Permanent codes (Mode 0A) cannot be erased with a tool. They age out after the ECU verifies the repair. If a permanent code remains, the monitor has not passed yet.
